Senior Community Worker — Centre & Community Rehabilitation (Full‑time) 

Ashgail Rehabilitation Services | Melbourne (South‑Eastern & Western Suburbs; opportunities across West/North & regional centres)

We’re expanding our community rehabilitation services and looking for a dedicated Senior Community Worker to join our team. If you’re passionate about supporting people to access allied‑health, build independence, and participate in community life — we want to hear from you.


Role snapshot

- Work type: Full‑time 

- Location: Melbourne (primary coverage South‑Eastern & Western suburbs). Occasional work across Melbourne West/North and regional centres (Bendigo, Ballarat, Gippsland) as required. 

- Expected salary: $85,000 – $90,000 + superannuation (negotiable based on experience) 

- Reports to: Practice Manager / Designated Senior Community Worker

Your role — key responsibilities

- Conduct psychosocial and functional assessments and co‑develop individualised support plans. 

- Coordinate access to allied health (OT, physio, speech, psychology, nursing) and community services; support NDIS pathway navigation.

- Provide advocacy, capacity‑building and practical supports to enhance independence and participation. 

- Monitor client progress, update care plans and contribute to multidisciplinary case planning. 

- Maintain accurate client records, outcome measures and compliance with NDIS Practice Standards and ARS policies. 

- Build and maintain partnerships with hospitals, community organisations and other service providers. 

- Deliver community education and contribute to service evaluation and continuous improvement. 

- Travel to client homes and community locations as required.


What we’re looking for

- Qualifications:Bachelor’s degree (AQF Level 7) in Social Work, Community Services, Community Development or related field. 

- Experience: Minimum 3 year experience in community‑based support, case management or service coordination (experience with complex needs and/or NDIS desirable). 

- Current: Victorian Driver’s Licence, Working with Children Check, and Victoria Police Check (pre‑employment). NDIS Worker Screening Check required or willingness to obtain. 

- Strong communication, report writing and stakeholder management skills. 

- Demonstrated ability to work independently, manage competing priorities and collaborate in MDTs. 

- Culturally responsive, trauma‑informed and client‑centred approach.
